What Is the Support at Home Program?
The Support at Home Program is Australia's new approach to in-home aged care, launched on 1 November 2025. It replaces the Home Care Packages (HCP) Program and Short-Term Restorative Care (STRC) Programme — with the Commonwealth Home Support Program (CHSP) set to transition no earlier than July 2027.
The program provides coordinated, person-centred access to personal care, clinical supports, domestic assistance, and — most relevant for Assistive Team clients — Assistive Technology and Home Modifications (AT-HM). Eligible products and modifications are defined in the official AT-HM List (also known as the ATHM List), published by the Australian Government Department of Health, Disability and Ageing. Supports are tailored to each person's assessed goals and needs, underpinned by the Aged Care Act 2024.
